Smart Goals Worksheet



I start off very motivated to reach my goals and lose steam after time.

I start off very focused on my goals but get distracted from managing my life.

My goals are vague and hard to measure. For example: “I want to lose weight,” as opposed to, “I want to lose 10 pounds by November 1.”

After a while, taking the action necessary to reach my goals becomes too much so I quit.

I procrastinate which is why I can’t achieve my goals.

If I had someone in my life who also wanted to reach a goal, it would be easier for me to stay on track.

I feel like the people in my life discourage me.

The critical voice in my head discourages me.

I start so many projects/goals at once that none of them get finished/accomplished.

The people in my life don’t consistently have goals for themselves.

I get so overwhelmed by my goals/the thought of goals that I become paralyzed.

I need a system for reaching my goals.
